Consent to the processing of personal data

Hereby I, further on – the "Personal Data Subject", to the satisfaction of the Federal Law No. 152-FZ d.d. July 27, 2006 "On Personal Data" (with amendments and additions) freely, with my will and in my interest give my consent to the Limited Liability Company "MEDLEX" (hereinafter referred to as "Local Authorities", address: (350010, Krasnodar, Zipovskaya St., 5, building 33) to my personal data processing, specified when filling out the web form on rutorika.ru (hereinafter referred to as the Site) and sent (completed) using the Site.

Under personal data I understand any information refering to me as a Personal Data Subject, including my last name, first name, patronymic, address, education, profession, contact information (phone, fax, e-mail, postal address), other information, which I see fit to turn in. Under the processing of personal data, I understand gathering, systematization, accumulation, refinement, updating, modification, use, distribution, transmission, including cross-border, depersonalization, blocking, erasure, permanent storage, and any other operations with personal data.

Processing of personal data of the Personal Data Subject is carried out solely for sending of e-mail messages by Local Authorities to the Personal Data Subject in response to request obtained from the Personal Data Subject.

The date, when the web form is sent from the Site, is the date of consent to the processing of personal data of the Personal Data Subject.

Processing of personal data of the Personal Data Subject can be carried out with or without automation facilities in accordance with the current legislation of the Russian Federation and internal regulations of Local Authorities.

Local Authorities takes necessary legal, organizational and technical measures or ensures their adoption to protect personal data from unauthorized or accidental usage, erasure, modification, blocking, copying, provision and dissemination, as well as from other illegal acts against personal data, and assumes the obligation to maintain the confidentiality of personal data of the Personal Data Subject.

I am aware that:

1. the present consent to personal data processing, specified in the request sent by the Site, is valid for 20 (twenty) years from the date of request;
2. the consent may be withdrawn by me based on a written statement in any format;
3. the provision of personal data of third parties without their consent involves responsibility in accordance with the current legislation of the Russian Federation.
